As an option to the online form, you may download and print, then complete ALL PARTS of the following form and submit it at an MMS Event and submit your fee payment by e-transfer or cash with the form.
This is less desirable than submitting the on-line form as it will take us more time to complete your application, issue you a Membership card, and notify you about events and include you in them, and it wastes paper/trees.

  MEMBERSHIP APPLICATION

  The following membership categories are available:

  • $15 per Student with valid secondary education identification
  • $20 per Senior age 60 years or older
  • $25 per Adult age 18 years or older
  • $30 per Family of 2 adults age 18 years or older and children under the age of 18 living at the same address
  • Membership fee may be adjusted for financially challenged applicants (submit email explanation)
  • Memberships expire on December 31 of the year they are purchased in, however
    • Memberships purchased between October 1 and December 31 expire December 31 of the following year
